AO-240 is the federal Administrative Office form used to apply for in forma pauperis status. It is a sworn financial affidavit covering employment income, other income, cash on hand, assets, dependents, monthly expenses, debts, and other circumstances.
The form must be signed under penalty of perjury. False statements are grounds for dismissal and may carry criminal penalties.
